Applying for Benefits
To apply for Health Care Reimbursement Program Coverage, you must be enrolled in a qualified health insurance plan.
You must show proof that you are enrolled in one of the qualified health insurance plans listed below. If you’re not currently enrolled in one of the plans below, you can enroll in one and then be apply for the Reimbursement Program.

*If you live in an area that an HMO health plan is not offered/ available, you may enroll in a EPO or PPO silver plan, whichever is lowest cost.
