You must apply for CCPU Health Care Fund benefits—they are not automatic.

Completing your initial application is simple:

Step 1: Provide your personal information (name, address, and provider license number, if you have one).
Step 2: Review your details and submit your application online or by mail.

Once your application is approved, you’ll receive:

  • Dental and vision coverage

  • Basic group life insurance

  • Provider Assistance Program (PAP) services (offering counseling and wellness resources.)

Want to access reimbursement benefits too?
Submit proof of coverage in a Qualified Health Plan with your application. If you don’t have proof ready, don’t worry—we’ll follow up with you to collect it later so you can extend your eligibility.
