Covering Your Dependents

When you join the CCPU Health Care Fund, you can include your eligible dependents in your benefits. Here’s how dependent enrollment works — whether you’re joining for the first time or you’re already participating

CCPU Health Family Benefits Plan Coverage

For New Applicants

If you are applying to the Fund (either via paper application or through our online portal):

  • You can add your dependents directly on your application form.

  • Simply enter their names and required details at the time of your initial enrollment.

  • Once your application (with dependents) is approved, your dependents will be covered for benefits such as:

    • Dental insurance

    • Vision insurance

    • Mental health benefits

You also have the option to purchase additional life insurance for yourself or your family members.

Because dependents are included from the start, there’s no need for a separate dependent form during your initial enrollment.

For Providers Already Enrolled in Health Care Fund Benefits

If you’re already part of the Fund and wish to add dependents later:

  • Use the Dependent Enrollment Addendum form to add dependents to your existing coverage.
  • Submit the completed Addendum as directed (e.g. via your portal, by mail, or email).
  • After approval, your dependents will begin receiving coverage just like yours (dental, vision, etc.).

You may also choose to enroll them in or expand life insurance options.

What Dependents Are Eligible?

Dependents typically include your legal spouse/domestic partner and children (under specified age limits). Be sure to review the Fund’s definition of “dependent” in the program rules or contact us to confirm eligibility.

To complete your application online, you’ll need proof of coverage for your health plan (a document showing your name as the policyholder, the name of your insurance plan, the coverage period, and the amount you pay for coverage).
