Your Basic Life Coverage

The CCPU Health Care Fund has partnered with Guardian x Legaci to bring you Basic Term Life insurance at no cost.

All Providers enrolled in the Health Care Fund in 2025 will receive these benefits automatically – There is no additional application needed!

Below is a snapshot of the benefits of the Term Life plan.

Basic Life
Provider Benefit CCPU Health Care Fund provides $50,000* Basic Term Life coverage for all full time providers.
Accidental Death and Dismemberment Your Basic Life coverage includes Accidental Death and Dismemberment coverage.
Guarantee Issue: The ‘guarantee’ means you are not required to answer health questions to qualify for coverage up to and including the specified amount, when you sign up for coverage during the initial enrollment period. Guarantee Issue coverage up to $50,000 per provider
Premiums Covered by the CCPU Health Care Fund if you meet eligibility requirements
Accelerated Life Benefit: A lump sum benefit is paid to you if you are diagnosed with a terminal condition, as defined by the plan. Yes, up to 50% of the death benefit
Benefit Reductions: Benefits are reduced by a certain percentage as a provider ages. No age reductions for providers

Providers enrolled in the Health Care Reimbursement Fund must submit documents designating beneficiaries. When filling out the beneficiary form please use group number 076878.

Filing a Claim after Death

1. If a provider has passed away, their designated beneficiary or family members must notify the Health Care Fund to begin the process. Please call 1-833-714-6028 to report the death. As the ‘Employer’, we have to initiate this process with the Life Insurance company, Guardian.

2. After reporting the death to us via phone, follow up with an email to us at [email protected], include your name, relationship to the provider, contact information, with a copy of the provider’s death certificate.

Add the following to the subject line: Provider Status Update – Deceased Documentation for immediate processing.

3. Beneficiaries/family must complete a claim submission form, which can be found here: Forms and Claims | Guardian

4. Beneficiaries/family must submit a completed claim packet to Guardian for next steps.

Beneficiaries FAQs
Is this a whole life insurance plan?

No. The group plans offered through the CCPU Health Care Fund are term life insurance only. As long as you are active in the Health Care Fund, providers don’t have a ‘term’ age/requirement. It comes into effect if a provider leaves the Health Care Fund and wishes to continue paying for the policy, subject to new rates and rules. Whole life coverage is not available under the group plan.

Can I keep my coverage if I leave the Fund or retire?

You cannot keep the free basic group life insurance ($50,000) if you retire or leave the Fund—it ends when your eligibility ends. However, if you purchased voluntary life insurance, you may be eligible to convert it into a whole life policy and take it with you.

What if I lose my subsidized child temporarily?

You are allowed a 3-month grace period (one quarter). If you lose a subsidized child, you have up to 3 months to regain eligibility by accepting another subsidized child. Your benefits will not be canceled during this time.

What’s included in the life insurance coverage?

This is true life insurance, not just accidental death coverage.

  • It does include benefits for accidental death and dismemberment
  • There is a list of qualifying accident types
  • In the event of death, your family would need to submit a claim form and a death certificate
  • If your death is an accidental death or dismemberment claim, your beneficiary may also need to submit a copy of a police report with your claim submission
What happens to the free $50,000 basic group life insurance when I retire or leave?

The basic $50,000 policy ends when you are no longer eligible (e.g., retirement, loss of subsidized child, or no longer in the Fund).

  • This policy is paid for by the CCPU Health Care Fund and only applies while you’re eligible
  • If you want continued life coverage, consider enrolling in voluntary life insurance, which you own and can convert or carry with you
What is the deadline to enroll in voluntary life insurance?

If you’re already enrolled in the Health Care Fund, the deadline to enroll in voluntary life insurance is June 30th. After that, you’ll need to wait until next year’s open enrollment period.

If you are not currently enrolled, you will have an open enrollment period of 30 days granted upon Fund program approval to elect coverage.

I’m not yet enrolled in the Fund. Can I still get these benefits?

Not yet. You need to:

  1. Meet the eligibility requirements
  2. Submit an application
  3. Enroll in the CCPU Health Care Fund

Once enrolled, you’ll have access to free group life insurance and the option to purchase voluntary life insurance.

What is a beneficiary?

A beneficiary is someone a Provider chooses to receive all or part of a death benefit.

  • There can be more than one beneficiary – and in practice, there often is.
  • A beneficiary doesn’t have to be a person – it can also be an entity such as a church, charity, or family trust
How do I designate a beneficiary?

Download this form and fill it out, and send back to the Health Care Fund. We will manage this information and in the event something happens to you, we will communicate your designations to Guardian.

What should Providers know about beneficiary designations?

In most states, if your provider names a minor beneficiary, we’ll need letters of guardianship for the property or estate of the minor to make the payout.

  • This typically requires the legal representative (e.g., surviving parent) obtain guardianship paperwork from the court.
  • Otherwise, if a claim is filed, the claim may be held for processing until the minor is of an ‘age of majority’ — 18 years old in most states.
  • Also, when the beneficiary is a minor, we cannot accept a funeral assignment (agreement allowing part or all of the proceeds to go directly to a funeral home). This may cause a financial hardship for the person who has to pay for the services.

 

If your Provider names a beneficiary who resides outside of the US:

  • The foreign bank may delay cashing a US check.
  • We may not be able to pay residents of high-risk countries sanctioned by the US.
  • Time differences may make it more difficult to locate the person or make contact.

 

The total primary beneficiary and contingent beneficiary percentages must each equal 100%.
